Job Objective:

Applies specialized knowledge and skills in the purchasing, storing, prescribing, assessing, preparing, dispensing and monitoring of pharmaceutical agents used in the treatment of acute care in collaboration with the multidisciplinary healthcare team for the purpose of optimizing healthcare outcomes under the direction of the Residency Program Director during the 12 months of the Pharmacy Resident Program.

Essential Responsibilities

1. Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations. 2. Follows assigned work schedule. 3. Provides oversight of Pharmacy operations, including technicians, regarding: Inventory management, Preparation and dispensing, Administration. 4. Manages drug therapy in accordance with P&Ps. 5. Accesses and uses computer systems effectively in managing patient care. 6. Processes medication orders accurately including medication reconciliation. 7. Ensures the safe use and timely delivery of medications. 8. Seeks team building opportunities with the healthcare members. 9. Interacts and communicates with the healthcare team and patients. 10. Applies critical patient care analysis skills to address therapeutic issues. 11. Assesses and ensures delivery of cost-effective pharmaceutical care through the optimal safe use of appropriate dosage regimens, patient allergies information, drug combinations, concurrent conditions, potential drug interactions and elimination of therapeutic duplications. 12. Prevents, detects, and reports adverse drug reactions, medication errors and unsafe medication use practices. 13. Educates patients and healthcare team members on the appropriate and cost-effective use of medications. 14. Respects and maintains confidentiality of patient records. 15. Complies with institution P&P and Code of Conduct policies. 16. Complies with industry standards of practice, and State & Federal laws. 17. Submits poster for ASHP Mid-Year meeting. 18. Completes four medication use evaluations during the program. 19. Implements one new medication. 20. Performs other duties as assigned.
