Advanced Practice Registered Nurse - Medicine, Cardiology - Full Time

The Advanced Practice Registered Nurse (APRN) delivers medical care to a wide variety of patients. The APRN is also examines and treats patients independently and in autonomous collaboration with other health care professionals. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. May prescribe medications and order diagnostic tests. Advises patients about continuing care.  

The APRN will participate in the Hypertension and Preventive Cardiology outpatient services at UHealth satellite locations. 

JOB FUNCTIONS  

  • Perform comprehensive health assessments and physical exams for new and follow-up patients. 

  • Develop and implement individualized treatment plans to control blood pressure, reduce cardiovascular risk factors, and manage related conditions (such as diabetes and hypercholesterolemia). 

  • Monitor medications, adjust dosages, and respond to side effects or complications. 

  • Evaluate global cardiovascular risk  

  • Provide lifestyle modifications and behavioral counseling to help control risk factors. 

  • Discuss medication options for primary and secondary prevention  

  • Order and interpret appropriate diagnostic tests. 

  • Monitor disease progression and complications through follow-up labs and imaging. 

  • Provide ongoing education about test results and health implications to help empower patient participation in their care. 

  • Provide patient education and resources to aid medication adherence and lifestyle modifications. 

  • Communicate effectively with other members of the care team to promote a collaborative approach. 

  • Engage in quality initiatives and education programs to help raise awareness and control complications related to cardiovascular disease. 

  • Reviews patient’s medications and reconciles them in the EMR.   

  • Orders lab tests, analyzes, interprets lab test results and takes appropriate actions. 

  • Enters orders. 

  • Requests labs and if labs not done or abnormal, takes actions including but not limited to: notify patient/family and MD and document it appropriately. 

  • Ensures that follow-up visit, MRIs and labs are appropriately scheduled.  

  • Responsible for writing visit notes on patients. 

  • Counsels patients and families on continued care. 

  • Responsible for the effectiveness of interventions, including but not limit to call patients with active issues until issues have resolved.   

  • First option for patient’s calls - test results, symptoms, med refills, authorizations, and forms. 

  • Be on charge of clinic flow, with the goal of see patients at the time they are scheduled and avoid clinic delays. 

  • Performs and documents complete physical examinations and comprehensive health histories.  

  • Functions independently to perform age-appropriate history and physical for patients. 

  • Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs. 

  • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities 

  • Implements interventions to support the patient to regain or maintain physiologic stability. 

  • Assists with the provision of care in accordance with facility, state, and federal regulations. 

  • Monitors the effectiveness of interventions. 

  • Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients. 

  • Collaborates with multidisciplinary team members by making appropriate referrals. 

  • Facilitates staff, patient and family decision making by providing educational tools. 

  • Adheres to University and unit-level policies and procedures and safeguards University assets.

QUALIFICATIONS  

 

Education: 

Master’s degree in relevant field 

 

Certification and Licensing: 

Valid State of Florida APRN License 

 

Experience:   

Minimum 2 years of relevant experience

  • Ability to exercise sound judgment in making critical decisions. 

  • Skill in completing assignments accurately and with attention to detail. 

  • Ability to analyze, organize and prioritize work under pressure while meeting deadlines. 

  • Ability to work independently and/or in a collaborative environment. 

  • Ability to communicate effectively in both oral and written form.
