JOB DESCRIPTION AND POSITION REQUIREMENTS

Scheduled Weekly Hours: 40

Work Shift: 8:00am to 4:30pm (Monday thru Friday)

Summary of the Job:
Supports the Quality and Population Health Program as the initial contact between our quality and population health initiatives and identified patients as part of an interdisciplinary team.  Responsible for reviewing, collecting, and analyzing encounters, and other clinical data required to measure and ensure quality standards of care are provided to all St. Tammany Physician Network (STPN) primary care patients.  Reviews patient charts and engages patients or caregivers in closing gaps in care, population health initiatives and coordinating PCP appointments via outreach and education.  Provides physician assistance and support by utilizing care coordination notes and communications to assist with closing gaps in care and compliance with prescribed treatment plan.  Documents in and updates the EMR as needed.  Provides formal activities report and updates to the Quality & Population Health Manager on a monthly basis.  Works closely with other decision support resources throughout STPN.  Maintains HIPAA standards and ensures confidentiality of protected health information.  Performs miscellaneous duties related to STPN quality and population health management as needed.

Minimum Qualifications

Graduate of a school of nursing and maintains a current nursing license issued by the Louisiana State Board of Nursing.  Strong computer skills required and must be proficient with Microsoft Office applications including Word, Excel and Outlook. Good working knowledge of ICD-10 and CPT Coding.

Preferred Qualifications:  

Strong healthcare background with at least 2 years’ clinical experience, ambulatory setting; knowledge of and experience with EMR systems a plus, Epic experience; Knowledge of and experience with HEDIS, CAHPS and Part D Safety measures.

Physical Demands:
Must possess good physical health.  Some requirements include but are not limited to standing, sitting or walking for long periods of time.  Lifting at least 10 pounds is required.  Must be able to work with a moderate level of noise.

Frequently (34%-66%) – NONE

Occasionally (1%-33%) - lifting, carrying, pushing/pulling, climbing, stooping, crouching, handling/feeling
