JOB DESCRIPTION AND POSITION REQUIREMENTS
Summary of the Job:
Serves as an integral part of the post-acute care strategy to effectively reduce hospital admissions/readmissions and potentially preventable Emergency Department (ED) visits. Responsible and accountable for the coordination of care for transitional care (post-discharge) patients. The Registered Nurse will assess and provide care as it relates to the specific needs of the patient within the framework of the State Nurse Practice Act.
Minimum Qualifications: Graduate of an accredited School of Nursing and three (3) years of experience in an acute clinical setting. Ability to demonstrate strong organizational, decision making, delegating, communicating and customer relations skills. Current R.N. licensure with Louisiana State Board of Nursing and current Basic Life Support (BLS) certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: Ochsner Health System, American Heart Association, and American Red Cross. The ILCOR course must include a manikin check off. ACLS and/or PALS, is preferred.
Preferred Qualifications: Bachelors of Science degree in Nursing. Critical Care, Emergency Care, and/or Home Health experience is a plus.
Physical Demands:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 50 pounds is required. Must be able to work with a moderate level of noise.

Frequently (34%-66%) - reaching
Occasionally (1%-33%) - lifting, carrying, pushing/pulling, balancing, stooping, crouching
