JOB DESCRIPTION AND POSITION REQUIREMENTS
Work Shift: M-F 8am - 4:30pm
JOB SUMMARY:
Licensed MSW who is responsible for Medical Social Services functions, evaluation, and counseling, for Palliative, Transition and Primary Wellness Home Care. Responsible for working with the Palliative, Transition and Primary Wellness Home Care staff to coordinate referrals, evaluation, counseling, acute/post-acute planning, long range planning, and patient care services, including Social Determinants of Health as indicated.
MINIMUM QUALIFICATIONS:
Acute Medical /Surgical, Rehabilitation and/or Crisis Inpatient/Outpatient Counseling, or Mental Health/Behavioral Clinics. Masters of Social Services degree by an accredited institution. Have licensure by examination or reciprocity by the Louisiana. Board of Certified Social Work Examiners.
Current BLS certifications through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: Ochsner Health System, American Heart Association, and American Red Cross. The ILCOR course must include a manikin check off.
PHYSICAL DEMANDS:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 20 pounds is required.

Frequently (34%-66%)- NONE
Occasionally (1%-33%)- lifting, carrying, pushing/pulling, climbing (stairs, ladders, etc.), balancing, stooping, reaching
