JOB DUTIES AND RESPONSIBILITIES:
Organizes and coordinates all activities and responsibilities for assigned community health initiatives, including entity specific community health program development, community health assessment implementation and strategic plan development following network-wide models as developed by the community health leadership. Navigates and ensures effective collaboration and implementation of the initiatives
Responsible for tracking outcomes as deliverables outlined by the community health scorecard
Assists with securing national, state and local grant funding, in partnership with community, government affairs and development to develop CHNA related initiatives at assigned campus and ensures community health program and grant implementation and reporting
Collaborates with all assigned department and entity-specific staff including interns, medical students and residents
Collaborates with community members, health professionals and hospital administrative staff to assure delivery of quality, cost-effective network-wide community health initiatives at assigned campuses
Assists Community Health and campus leadership to build coordinated network community health initiatives and advocate for hospital administrative support
Complies with all requirements of grant funding and submits required reports in a timely manner. Actively seeks new grant opportunities to support the growth of network initiatives. Assist with grant writing to obtain funding
Monitors compliance with standard policies and procedures
Collects and organizes grant data to monitor effectiveness of assigned Network Community Health Initiatives, and submits reports highlighting effectiveness of programs. Submits periodic reports to the leadership, as required
Organizes, coordinates, and prioritizes patient care consistently utilizing available resources
PHYSICAL AND SENSORY REQUIREMENTS:
Sit up to 4 hours per day; 2 hours at a time. Stand for up to 6 hours per day 3 hours at a time. Walk 6 hours per day; 20 minutes at a time. Consistently lift, carry and push objects up to 10 lbs. Frequently stoop and bend. Frequently reach above shoulder level. Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation, and high and low frequencies. Must be able to see as it relates to general, near, far, color and peripheral visions
EDUCATION:
Master’s or Bachelor’s degree in Public Health or related field preferred or Associate Degree plus 8+ years of related healthcare community health required.
TRAINING AND EXPERIENCE:
At least three years experience coordinating community/public health programs. Ability to be an effective communicator (oral and written) build, engage, maintain, and sustain constructive relationships with a diverse population. Ability to align and meet the identified outcomes and deliverables in the community health strategy and scorecard.
