Cotswold Family Medicine is seeking a full-time Physician Assistant or Nurse Practitioner to join their growing team in the heart of Charlotte, NC. Ideal candidates will be board-certified and have experience with all ages of patients and the breadth of a typical FM practice.

For over 20 years, Cotswold Family Medicine has been serving central Mecklenburg County, including the surrounding neighborhoods of Myers Park, Southpark and Eastover. Cotswold Family Medicine is proudly connected to the community and annually participates with the Cotswold Farmers Market to promote the importance of preventive screenings.

Position Highlights:

• 100% outpatient
• Centrally located in the Cotswold Community, quick access to Uptown Charlotte
• Join current team of 2 physicians and 1 ACP
• Established patient panel
• Shared call - rotating schedule
• Collegial relationships that foster a team approach to patient care and a family environment at work
• Available on-site: Chronic Care Management, PharmD and CCPGM Care Managers who have access to social work, dietitians and finical counseling
