Pay Range
$30.15 - $45.25Join a dynamic and innovative Lifestyle Medicine practice that is redefining patient care! Originally launched as a pilot program in Cabarrus, we became a standalone practice in March and are growing rapidly. Our mission is to partner with patients beyond the traditional 15-minute primary care visit, helping them manage chronic conditions through lifestyle changes, trauma-informed care, and behavioral health support. If you’re passionate about empowering patients to reduce reliance on medication and achieve sustainable health goals, we’d love to have you on our team.
Essential Functions
Provide independent psychotherapy services to patients enrolled in the Lifestyle Medicine program.
Collaborate with physicians, APPs, and behavioral health mentors to deliver holistic care.
Conduct individual therapy sessions (weekly, biweekly, or monthly) tailored to patient needs.
Support patients in addressing barriers such as trauma, eating disorders, and behavioral challenges.
Participate in virtual visits, home visits, and group sessions to foster patient engagement and peer support.
Document patient progress and communicate updates to primary care providers.
Promote lifestyle interventions aimed at reducing or eliminating medication use.
Maintain compliance with organizational standards and best practices provided by the Behavioral Health Division.
Provide therapeutic support and interventions for adolescent patients, demonstrating comfort and skill in engaging with youth populations.
Deliver presentations with confidence in virtual and in-person settings.
Physical Requirements
Ability to perform virtual and in-person visits, including occasional home visits.
Must be able to sit, stand, and move between patient locations as needed.
Requires sufficient mobility and dexterity to operate standard office and telehealth equipment.
Education, Experience and Certifications
Master’s degree in counseling, psychology, social work, or related field.
Independent licensure required (LMFT, LCSW, LPC, or equivalent).
Minimum of 2 years of clinical experience providing therapy services independently.
Prior experience in trauma and food addiction required.
Familiarity with trauma-informed care and chronic condition management preferred.
