JOB PURPOSE:
The Regional Home Care Recreation Therapist is responsible for the development and implementation of the participant therapeutic recreation services at multiple settings within a geographic region. The recreation services include but are not limited to completion of clinical assessments, and facilitation, coordination, and scheduling of therapeutic recreation programs.
The Regional Home Care Recreation Therapist will oversee and support the delivery of high-quality recreational therapy services to patients in various settings, such as, but not limited to clinics, home health, or outpatient services, under the direction of Director of Recreation. In addition, this role will have a significant involvement in auditing and quality reviews. The regional Home Care Recreation Therapist will be responsible for making recommendations to Recreation Management regarding service delivery and documentation of services.
JOB RESPONSIBILITIES:
Direct Participant Care:
Programming
- Prepare a monthly recreation calendar for the DHC and submit to Recreation management by the first Wednesday of the month to maximize the Participant experience.
- Schedule and implement programs that address the cognitive, physical, emotional and leisure needs of participants. Schedule should be posted and translated into the appropriate languages.
- Conceptualize and Implement participant educational programs using either CenterLight Employees or outside experts to enhance a Participant’s quality of life.
- Schedule and facilitate trips to destinations of interest to the participants. Arrange for any prior approval of expenditures with Recreation Management
- Decorate the DHC for seasonal and holiday themes
- Encourage participants to activity participate in programs while monitoring their safety.
- Document DHC events with photos and videos (with participant consent) and share with the Marketing Dept as well as at the DHC.
- Clean up after recreational programs
- Monitor for participant safety during activities
- Facilitate annual Participant Advisory Council (PAC) representative elections and assist with encouraging attendance at quarterly meetings
- Conduct an orientation for new participants to the DHC. Provide them with a tour of the DHC, share the location of CLHC participant Bill of Rights, share program calendar, provide hours and contact information, show them the various activities available, and introduce them to Site employees.
- Encourage participants to come to the DHC, input DHC orders in relevant systems, arrange transportation, update member aide services, and coordinate with other IDT members.
- Report to the IDT at morning report cancellations / absences of participants
DHC Management:
- Create and distribute ID badges for DHC participants as needed
- Posting of all mandatory posters, notifications, signage of changes made to the recreation calendar
- Monitor and maintain inventory of recreational supplies and equipment. Ensure equipment is maintained and in good working condition. Initiate orders for repair or replacement as required.
- Store recreational supplies appropriately. Ensure necessary storage cabinets/closets are locked when not in use to ensure participant safety.
- Performs general maintenance such as: setting up/storage of equipment, Receive invoices / packing slips from recreation supplies delivery and DHC entertainers and forward to Recreation Management.
- cleaning/dusting of equipment as needed, maintaining cabinets and storage areas, counting/ordering supplies, and counting and recording daily census.
Participant Assessment:
- Performs participant screens, assessments, and care planning following as required by CenterLight Policies and Standard Operating Procedures.
- Ensures careplan and servie plan is updated semi-annually to reflect DHC, SADC and SADC transporation orders x6 months
- Create x6month transportation orders for DHC and SADC for internal transportation coordinators.
- Participate as a member of the Interdisciplinary Care Team (IDT).
Compliance and Quality Assurance:
- Assist Director of Social Work in ensuring that all therapy services comply with state and federal regulations, accreditation standards, and company policies.
- Assist Director of Social Work in monitoring and reviewing patient records, documentation, and progress reports to ensure accuracy and completeness.
- Participate in quality improvement initiatives and audits to enhance patient care and operational efficiency.
Administrative Responsibilities:
- Attend and actively participate in IDT and care plan meetings. Identify and document, as part of the care plan, and present recreation-related concerns and/or issues and outline and discuss appropriate recreation interventions and recommendations. Ensure and manage recreation interventions are applied by recreation staff and participant-centered goals are met.
- Represent the Recreation department and CenterLight at appropriate meetings and conferences at the site level and in the community.
- Participate in appropriate continuing education and professional training programs sponsored internally by CenterLight and/or applicable outside agencies. Participate in staff enhancement through professional knowledge by attending approved seminars/workshops relevant to the field of recreation.
- Participate in mandatory in-service education provided by CenterLight.
- Ensure documentation is completed timely (within 24-72 hrs) and accurately.
- Only act within the scope of the individual’s authority to practice.
- Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.
- Acting member of the IDT.
Other duties as assigned
- Will provide supervision of recreation services in clinic and field as necessary
- Will provide orientation and clinical guidance to new team members as needed
Schedule: 8:30 AM – 5:30 PM
Weekly Hours: 40
QUALIFICATIONS:
Education: Bachelor’s Degree in Recreation or related field required.
Experience:
- Three to five (3-5) years of experience in Recreation.
- Experience in working independently without significant oversight and or direction from a supervisor.
- Minimum of one (1) year of experience working with the elderly
Other:
- Must obtain and maintain a New York City Food Protection Certification within six months of hire. Must maintain certification throughout employment.
- Positive attitude and outlook with the ability to multi-task in a constantly changing environment.
- Flexibility one weekend to meet deadlines.
- A highly motivated and self-driven individual willing to work within a strict deadline environment.
- Proficiency in Microsoft Office products; report development and strong writing skills
- Excellent communication and interpersonal skills are required.
- Customer Service-oriented with clinical solid management skills
- Bilingual skills are strongly preferred.
Physical Requirements
Individuals must be able to sustain specific physical requirements essential to the job. This includes, but is not limited to:
- Standing – duration of up to 6 hours a day.
- Sitting/Stationary positions – sedentary position for up to 6-8 hours a day for consecutive hours/periods of time.
- Lifting/Push/Pull – up to 50 pounds of equipment, baggage, supplies, and ability to lift patients safely, using OSHA guidelines, etc.
- Bending/Squatting – have to be able to bend or squat to care for patients safely, use medical supplies, etc.
- Stairs/Steps/Walking/Climbing – must be able to maneuver stairs safely, climb up/down and walk to access work areas Position requires an individual to be able to travel, walk between sites/locations and work areas throughout the day.
- Agility/Fine Motor Skills - Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools (i.e., typing, use of medical supplies, equipment, etc.)
- Sight/Visual Requirements – must be able to visually assess patients, read orders and type/write documentation, etc., accurately.
- Audio Hearing and Motor Skills (language) Requirements – must be able to listen attentively and document information from patients, community members, providers, etc., and intake information through audio processing with accuracy. In addition, must be able to speak comfortably and clearly with language motor skills for customers to understand the individual.
- Cognitive Ability – Must be able to demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job.

Salary Range (Min-Max):
Salary Range: $75,000 - $85,000/yr.
