Job Description:

1.Referrals and Admissions
1.Evaluate clinical records for referrals same day as referral, unless otherwise indicated.
2. Provide timely and consistent feedback to referral sources, physicians, case managers, patients/families during the referral process.
3.Will refer patients not taken under care to Palliative Care Program or other appropriate referral resource when appropriate.
4.Will follow up on patients not taken under care on a monthly basis and report to IDT.
5.Completion of the Hospice Election forms, Initial Comprehensive Assessment, and plan of care that addresses the patient's immediate needs.
6.Communicate accurately and completely with all members of the IDT, Medical Director/Physician Designee/Attending Physician regarding all referrals and admissions.
7.Completion of accurate and timely documentation in Hospice EMR and Hospital EMR when appropriate.
2.Community Liaison
1.Educate case managers, physicians, ALFs, SNFs and other referral sources to facilitate timely referrals.
2.Acts as a liaison between Hospice of Scotland County and contracting agencies to provide education on hospice and palliative care services provided by our agency.
3.Maintain regular contact with hospital case managers/discharge planners, physicians, and other staff to provide ease of access to hospice services.
4.Assists with the development of marketing and educational materials tailored for the referral sources.
5.Develops and maintains a referral database with the Director of Marketing of all scheduled visits, contacts made and follow up activity.
6.Assists in collecting feedback from referral sources.
7.Assists in monitoring the effectiveness of the marketing efforts and activities
3.Other Duties as Assigned by Director of Homecare
1.Attends IDT meetings acting as a consultant to other staff.
2.Makes PRN and regular home visits as directed.
3.Assists Hospice Case Managers and other members of the IDT with contacting physicians for needed orders, treatments, and changes to Plan of Care and documents in EMR appropriate orders and changes to Plan of Care.
4.Assists with providing 24 hours on call services as scheduled.
5.Other duties as assigned.
