EMPLOYMENT RELATIONSHIP: Just cause subject to probationary period
CURRENT HOURS AND STARTING TIME: Full-time (37.5 hours per week) position. The starting time for this position is 8:30 a.m. and may require flexibility to work outside of normal working hours and attend community events.
GENERAL SUMMARY
Under direction, identifies resources and establishes connections within the region for veteran and military families. Develops strategies and plans to enhance the accessibility of programs for those being served. Provides recommendations, coordinates care, conducts assessments and intakes, and ensures wraparound services are either directly provided or made accessible to veterans and military families.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Performs preliminary assessments and formulates action plans to fulfill the needs of veterans and military families.
Connects veterans and military families with programs and services within their local community that best meet their needs.
Facilitates access to services such as referrals for behavioral health evaluation and therapy, peer support, or other community-based resources.
Collaborates with volunteers to enhance the coordination of care and strategic planning.
Evaluates the quality of service and efficiency of veteran organizations in delivering assistance.
Assists in the development of a regional work plan that optimizes the implementation of services and aligns with the goals of the veterans and military members strategic plan.
Promotes county, state, and federal services and programs that are accessible to veterans and military families.
Maintains communication with veterans, military families, care providers, and service agencies for seamless transitions in service delivery.
Collaborates with representatives of local and regional organizations such as the Military and Veteran Affairs Administration (MVAA), Veterans Health Administration (VHA), Veterans Benefits Administration (VBA), and local county Veteran Affairs (VA) to coordinate the provision of services.
Ensures coverage and delivery of services by providing and coordinating follow-up on all veteran and military family contacts.
Offers seamless transitions for veterans and military families to participate in evidence-based programs within the behavioral health services network.
Attends quarterly veteran navigation meetings and other conferences focused on veterans, behavioral health, and initiatives for veteran peer support.
Ensures veterans and military families are informed about the educational opportunities available in the area that can support their recovery, reintegration, and resilience.
Maintains, reviews, and compiles data to summarize essential details for quarterly reporting obligations.
Submits quarterly reports to help measure outcomes, identify treatment barriers, and monitor trends among veterans and military families across the state.
Operates an automobile to perform assigned job functions.
May include evening or weekend work.
Performs related duties as assigned.
QUALIFICATIONS
Required Education and Experience
Master’s degree from an accredited college or university in Social Work, Psychology, Special Education, or mental health-related field
Must be a veteran, service member, spouse of a veteran, or immediate family member of a veteran
One (1) year of experience working with community organizations, evaluating, assessing, planning, developing, and implementing programs or services
One (1) year of experience providing services to veterans and military families
Preferred Education and Experience
Two (2) years of experience working with community organizations, evaluating, assessing, planning, developing, and implementing programs or services
One (1) year of experience or more working in volunteer management
Required Licenses or Certifications
Licensed Master Social Worker (LMSW), Limited Licensed Master of Social Work (LLMSW), Licensed Professional Counselor (LPC), Limited Licensed Professional Counselor (LLPC), Temporary Limited Licensed Psychologist (TLLP), Limited Licensed Psychologist (LLP), or Licensed Psychologist (LP)
Possession of state licensure appropriate to position and ability to meet privileging and credentialing guidelines
Possession of a valid Michigan driver’s license and operable, insured automobile for authorized travel
Preferred Licenses or Certifications
Certification with the Michigan Certification Board for Addiction Professionals (MCBAP) or an approved development plan
