The Pharmacy provides pharmaceutical services to patients across the continuum of care. Services include, but are not limited to verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation and a meds to beds program for inpatients.
The Ambulatory Pharmacy Specialist is responsible for providing clinical pharmacy services for ambulatory patients through accurate medication reconciliation; developing patient care plans including the selection, dosing, and monitoring of drug therapy; promoting medication adherence; and educating patients and other health care providers regarding complexities of drug therapy. Other activities may include ordering labs, changes through collaborative care with clinic providers. The position will require working within a multidisciplinary team and supporting patient transitions through the continuum of care. The Ambulatory Pharmacy Specialist supports the development, implementation, and maintenance of programs that promote desirable patient outcomes through safe, appropriate, cost-effective medication therapies. Delivers a high level of service to each patient in every interaction, ensuring they perceive Baptist Health’s pharmacy as the pharmacy of choice and ultimately impacting their level of success in their therapy. Ensures compliance with pharmacy regulatory standards. This pharmacist may also work on other projects as assigned. The scope and complexity of the position requires a diverse and highly developed skill set including but not limited to time management, communication, creativity, vision, teamwork, management, leadership, clinical knowledge base/skills, mentorship, change management, scholarship and teaching. Candidates must be able to work effectively with a multidisciplinary team, handling multiple responsibilities in a fast-paced environment. This role will be physically positioned in an ambulatory medical clinic setting when possible.
Job Description:
Responsibility: Clinic Integration and Coordination
Essential Duties
Collaborate with providers and clinical staff to integrate pharmacy services into patient care.
Communicate with clinicians to support smooth coordination for new and existing patients.
Maintain clinic pharmacy operations, policies, procedures, and workflows.
Represent pharmacy on multidisciplinary committees and teams.
Educate clinic and pharmacy staff through trainings and in-services.
Address inquiries from staff, providers, patients, and families.
Ensure compliance with regulatory and accreditation requirements
Responsibility: Patient Care Services and Operations
Essential Duties
Perform medication reviews and provide clinical recommendations to optimize therapy.
Educate patients and conduct ongoing assessments, including medication reconciliation.
Manage medication therapy and disease states; coordinate therapy adjustments.
Document patient care activities in the electronic medical record.
Provide pharmacokinetic monitoring and assess drug interactions.
Conduct point-of-care testing when applicable.
Identify adherence and access barriers; support refills and follow-up needs.
Refer patients for financial assistance programs as appropriate.
Practice continuous quality improvement and patient-centered care.
Communicate benefits of filling prescriptions with Baptist Health.
Document billing codes when applicable.

Precept pharmacy students and residents as applicable.
Requirements: • Minimum of Bachelor's degree with one year of experience. Kentucky and/or Indiana Board of Pharmacy license in good standing.
