Compensation Range:

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE:

Accountable for the provision of proficient patient care inclusive of complex patients. Assumes an expanded charge nurse role and assumes unit responsibility for orientation for new staff. May assume the role of care coordinator for a given group of patients, monitoring and evaluating patient care based on the unit standards of care. Actively involved in unit/service activities, contributing to outcomes from those groups.

FT/PT positions available, D/E/N, 12 hour shifts, holidays and weekends per program policy.   

 Exciting opportunity to join a new program. This program offers an innovative approach to acute care nursing in our communities. Patients enrolled in Hospital at Home will require an inpatient level of admission which will be brought to their homes rather than in a traditional hospital unit. Joining the Hospital at Home team is a rare opportunity to be part of opening and expanding a groundbreaking care model.  This program has been newly launched and is looking for experienced, forward-thinking and creative RNs to pioneer it. Hospital at Home nurses will be trained for and fill a variety of duties including charge RN, field RN and triage RN. Responsibilities include performing remote clinical triage, remote & in person patient assessment and administration of therapeutic interventions in patients' homes. H@H RNs will have the opportunity to develop a diversified set of skills. Individuals must possess strong clinical assessment skills, effective communication skills and ability to adapt to rapid changes. H@H offers opportunities for professional growth while working with multiple disciplines in a team focused environment. The ideal candidate has enthusiasm for learning, patient-centered care and working in a team. Minimum 2 years of acute care, ICU and/or ED experience required. 5+ years of broad clinical expertise is preferred. This position requires a valid NYS driver’s license and reliable transportation.
