Compensation Range:

$25.09 - $35.12

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE
Responsible for the assessment of patient strengths and weaknesses in the areas of living skills and vocational/occupational functioning, and for the planning and implementation of individual and group strategies to address deficits in areas of patient functioning. Serves as a liaison from the CPS program to other community agencies involved in vocational and living skills development.

ESSENTIAL FUNCTIONS

  • Conducts a comprehensive evaluation of the rehabilitation and social support needs of clients and plans.
  • Coordinates and implements individual and group activities designed to enhance patient skills in the areas of food preparation, shopping, cooking, living skills, housekeeping, laundry, vocational functioning, etc.
  • Supervises and evaluates patient work groups in various living skills and in pre-vocational workshop areas.
  • Prepares a written assessment of client skills, deficits and growth potential and maintains written progress notes in the medical record.
  • Participates in program planning and initiates the development of new rehabilitation therapies based on a needs assessment.
  • Provides feedback to the treatment team regarding patient progress and facilitates and supervises the placement of patient in community living and work environments.
  • Serves as liaison between the CPS program and other community agencies, such as the health department, the Office of Vocational Rehabilitation and Sheltered Workshops.
  • Orders and purchases supplies for the kitchen, company store and prevocational areas.
  • Determines menus and/or work projects for prevocational training.

MINIMUM EDUCATION & EXPERIENCE

  • Bachelor's degree with major course work in a related health/occupational field and 2 years of relevant experience working with the mentally ill required
  • Or equivalent combination of education and experience
