$20.99 - $28.34The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
Provides professional comprehensive care coordination services to patients of the Golisano Children’s Hospital. Collaborates with health, behavioral health and social service providers and is responsible for assessing patient’s needs, enrolling new patients in the Complex Care Program, and coordinating supportive services as needed. Special focus will be serving the most complex, high risk patients enrolled in the Complex Care Program. Services include, but are not limited to: care coordination, heath promotion, comprehensive transitional support, enrollee and family support, referral to community and social supports, use of technology to link services.ESSENTIAL FUNCTIONS
