Compensation Range:

$76,398.00 - $99,320.00

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE:

Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.

FT (40 hours) position available: Monday – Friday, day shift. Some remote.

URMC GI Practice is in search of a driven, team-oriented nurse to join our multidisciplinary team providing focused, sub-specialty care to our IBD (Inflammatory Bowel Disease) patient population. This role helps patients navigate the complex medical needs in the management of Ulcerative Colitis and Crohn’s Disease. The IBD Nurse Navigator will develop and implement a care management plan based on patient/provider developed goals and objectives to promote improved health care outcomes and quality of life.  This role involves managing a list of high-risk IBD patients, monitoring medication trough levels, managing infusion schedules, providing education and support, and telephone triage/ in-basket management. The ideal candidate will have a Bachelor’s degree and 2-3 years GI nursing experience with strong organizational and communication skills as well as experience working within the electronic medical record. The IBD Nurse Navigator will work effectively as a member of the health care team by collaborating on population health management, complex care and transitional care management.
