$77,210.00 - $104,229.00The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
General Purpose: Provides cancer disease-specific individualized assistance, support, and comprehensive coordination of care for patients and their families/caregivers to eliminate barriers to timely care, facilitate flow through the system, facilitate interactions with the care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as the point of contact for patients and families throughout the cancer continuum. Works in collaboration with the care team and other multidisciplinary teams to establish, execute, and evaluate a care plan across the system for URMC patients.FT position available, primarily days, 8-hour shifts
Seeking a team-focused professional, with a passion for caring for oncology patients, to join our progressive outpatient clinics. The Oncology Nurse Navigator identifies patient needs, barriers to timely care and initiates appropriate referrals to multidisciplinary services from diagnosis through treatment. The navigator collaborates with the appropriate teams to ensure that scheduled tests, diagnostic procedures, follow- up care, and appointments are coordinated within the cancer clinical pathway. The navigator provides education to patients and families regarding pre-appointment management and the determined plan of care. Excellent communication skills and the ability to work independently with multidisciplinary teams required to be successful in this position. BSN with 3- 5 years of oncology experience required. Oncology certification preferred.
