Responsibilities:
GENERAL PURPOSE:
Accountable for the provision of proficient patient care inclusive of complex patients. Assumes an expanded charge nurse role and assumes unit responsibility for orientation for new staff. May assume the role of care coordinator for a given group of patients, monitoring and evaluating patient care based on the unit standards of care. Actively involved in unit/service activities, contributing to outcomes from those groups.
ESSENTIAL FUNCTIONS - Provides patient care in an expert manner, following policies and procedures. Performs ongoing comprehensive assessments, including physiologic, psychosocial, and developmental components. Documents assessments, provision of care, medication administration, patient/family education, patient/family responses, and care planning according to unit and service standards. Identifies complex patient problems and collaborates in developing a plan of care based on evidence from the literature, best practice standards, and patient goals. Implements age-specific, culturally, and spiritually competent nursing interventions to address patient/family needs and response to procedures, therapies, and medications. Recognizes trends or subtle changes in patient/family response to care and responds effectively. Differentiates unique or complex age specific patient/family learning needs and barriers to learning. Implements an educational plan based on identified needs, as well as cultural and spiritual considerations. Monitors staff compliance with unit and service documentation standards. - Resolves conflict positively and in a timely manner, working directly with co-workers to address issues. Uses conflict management strategies to continuously improve staff relationships/unit operations. Treats patients, family, and staff with kindness, courtesy, respect, and trust. Demonstrates respect for diverse view points and cultures. Communicates effectively in written, verbal, and non-verbal interaction with healthcare members, patients and families. Listens attentively to customers, paying attention to both verbal and non-verbal messages. Communicates with customers in a clear, courteous, caring, and appropriate manner to the situation. - Demonstrates responsibility for continuous learning to improve self and performance, including progress toward achievement of annual goals. Functions effectively as an expert charge nurse and preceptor for staff and students. Provides learning opportunities and contributes to the orientation of team members and students. Demonstrates good communication, fairness, and support for staff while modeling an approachable, calm, helpful, and efficient demeanor. Accepts and acts on constructive feedback from leadership, peers, and colleagues in a positive manner. Demonstrates flexibility in scheduling and assignments to meet patient and unit goals. Identifies potential problem areas and independently initiates problem-solving process in collaboration with unit leadership as appropriate. - Reviews and evaluates evidence from literature and best practice standards for application to patient and unit needs. Assists with the collection of data to support unit performance improvement activities. Assists leadership in implementing changes identified through performance improvement needs. - Other duties as assigned. MINIMUM EDUCATION & EXPERIENCE - Bachelor's degree in Nursing Or Actively Pursuing a BSN (minimum 6 credits or more towards degree) and 2 years experience as a Registered Nurse working in an acute care setting required - Or equivalent combination of education and experience KNOWLEDGE, SKILLS AND ABILITIES - Successful completion of orientation at the unit level required LICENSES AND CERTIFICATIONS - RN - Registered Nurse - State Licensure and/or Compact State Licensure in New York State (NYS) upon hire required
