The Resident Care Coordinator is responsible for coordinating client care related to scheduling of and communication following appointments, transportation, and insurance. In addition, this position guides and assists individuals to live and function as independently as possible in the community and participates in the development and implementation of individual program plans and coordination for the provision of specialized services. This position requires the position to travel between sites on a regular basis. This position requires at least an associates degree with 4 years of work experience, OR a bachelor's degree with 2 years of experience, OR a masters degree and 1 year of work experience.Schedule: Mon - Fri 8am - 4pm, depending on Lab work and other scheduled doctor appointments.
