The IMPACT Site Lead oversees the implementation and daily operations of a clinical screening program at Jefferson Philadelphia area Emergency Department(s). Current clinical screenings include HIV and Hepatitis B & C, with potential for expansion. This role is responsible for supervising navigator duties, coordinating patient follow-up, managing and aggregating screening and outcomes data, supporting on-site screening activities to ensure timely linkage to care and partner services, and contributing to overall program quality improvement. The Site Lead directly supervises IMPACT Patient Navigator(s), providing day-to-day oversight, guidance, and support. The Site Lead works closely with the IMPACT Patient Navigators, Data Analyst, clinical teams, and community partners to maintain high-quality data collection and support continuity of care for patients identified through screening. Additionally, this role provides leadership to ensure consistent workflows, performance accountability, and alignment with program goals.

Job Description

Essential Functions:

  • Conduct follow-up outreach for screened patients, with a focus on those who screen positive, to support communication of results, confirm next steps, and facilitate timely linkage to care.

  • Manage and oversee day-to-day data collection, including screening volume, results, patient demographics, linkage-to-care activities, medical care engagement, and referrals to internal and external partner services. Ensure accuracy, completeness, and compliance with program and funder requirements.

  • Troubleshoot workflow challenges and collaborate with other Site Leads and program staff on program improvement initiatives.

  • Collaborate with other IMPACT teams to ensure coordination of initiatives, sharing of best practices, and identification of new opportunities.

  • Supervise up to 2 IMPACT Patient Navigators, who conduct bedside screening and education, by providing day-to-day oversight, guidance, and support.

Additional Responsibilities:

  • Coordinate with social work, care navigators, service line leads, clinical teams, and specialty programs (e.g., oncology, infectious disease, hepatology, pulmonary), among others, to ensure patients with positive results are successfully connected to medical care and recommended follow-up services.

  • Track, trend, and analyze site-level screening and outcomes data to monitor program performance, identify gaps or opportunities for improvement, and support quality improvement efforts.

  • Support preparation of regular site-level data reports and summaries for internal stakeholders, service line leadership, and external funders, as required.

  • Maintain regular communication with IMPACT staff and other relevant stakeholders.

  • Attend scheduled staff meetings and trainings and participate in discussions.

  • Operate with a high level of independence, as staff will often be working with limited team members onsite across Enterprise locations.

  • In many locations, this will be a new workflow that requires ongoing attention to achieve clinical staff buy in and partnership. This staff member will play a critical role in building relationships and influencing buy in.

Competencies:

  • Strong communication and interpersonal skills, with the ability to engage with diverse populations

  • Comfort with public speaking

  • Proficiency in use of Microsoft Word, Excel, and email required

  • Database management experience required

  • Experience in supervisory role preferred

  • Familiarity/proficiency with applications such as EPIC, REDCap, and basic statistical packages (SPSS, SAS, STAT) preferred

  • Bilingual skills (English, Spanish) preferred

  • Phlebotomy experience preferred

Minimum Education and Experience Requirements:

  • A Bachelor’s degree required, and preferred that the degree be in public health, medicine-related track, health education, nursing, social work, healthcare administration, or a related field.

  • Preferred Master’s degree, particularly in nursing, social work, public health, or a related field

  • At least 1 year of experience working in a clinical environment (including as volunteer/intern).
