Summary
The Facility Activation and Transition Planning Manager supports a diverse portfolio of initiatives within the Enterprise Asset Management department, including HOPD conversions, facility activations, decommissioning, and move management. This role partners closely with leaders across the organization to develop and execute project plans aligned with strategic and capital objectives. Success in this position requires strong project leadership, exceptional communication skills, and the ability to manage confidential, complex, and fast‑moving initiatives.

Job Duties

  • Lead end‑to‑end activation planning and execution for HOPD conversions, facility activations, decommissioning, move management, and other key initiatives.
  • Engage stakeholders, define project teams, and secure required approvals, resources, timelines, and budgets.
  • Develop and communicate project objectives, deliverables, and status updates; present plans and outcomes to leadership and cross‑functional partners
  • Leverage data and root‑cause analysis to assess performance gaps, drive efficiency, and support strategic decision‑making.
  • Build strong relationships with internal partners and vendors; manage RIFs/RFPs, contracts, and onsite vendor oversight.
  • Support departmental strategies, lead continuous improvement efforts, and maintain awareness of healthcare and project management best practices.
  • Conduct lessons‑learned and financial lookbacks; maintain complete project documentation and travel onsite for key milestones.


Minimum Qualifications

  • Bachelor’s degree in a relevant field OR 3+ years of progressive project management experience OR completion of a healthcare administrative fellowship.
  • 3 years of progressive project management experience or completion of a healthcare administrative fellowship
  • Proficiency with Microsoft Excel, PowerPoint, and project management tools.
  • Demonstrated ability to manage multiple complex projects in a fast‑paced environment.
  • Strong analytical skills with the ability to extract, interpret, and leverage data from various systems.
  • Excellent attention to detail, organizational skills, and ability to prioritize effectively.
  • Strong interpersonal and team‑building skills, including emotional intelligence.
  • Effective communication, problem‑solving, and negotiation abilities.


Preferred Qualifications

  • Master’s Degree in a related field
  • 3 years experience in a healthcare setting, facility activation, HOPD, move management a plus.
  • Comfortable leading large group meetings virtually and in person.
  • Ability to quickly grasp complex concepts with minimal supervision.
  • Demonstrated resilience, flexibility, professionalism, and managerial courage.


Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting
