Education: Bachelor's degree in social service field or a relevant degree.
Experience: Three years of progressive employment experience working with individuals with intellectual or developmental disabilities. Training and/or experience in developing and implementing active treatment programs, direct service delivery, staff training and case management.
Supervises: No Supervision Component
Summary: The QIDP/case manager is responsible for using sound professional judgment and best practices to ensure quality case management services are provided to the service population especially in crisis situations while maintaining compliance with established state and federal standards.

Essential Functions:
1. Perform the various duties required of a QIDP (Qualified Intellectual Disabilities Professional) including developing, monitoring and revising active treatment goals and objectives; conducting annual Interdisciplinary Team Meetings (IDT) and interim IDT meetings for residents; writing quarterly, interim, and annual reports for each resident.
2. Ensures compliance with ICF-IID contract standards, state licensing requirements, regulating/accrediting bodies and BCFS policies/procedures.
3. Ensures that all documentation is completed appropriately and timely and client documentation in case files.
4. Participates and assists in in-service training.
5. Provide appropriate social services in accordance with licensing standards, contract terms, agency policies, ICF standards and funding requirements.
6. Supervise, facilitate and participate in the resident assessment and admission process.
7. Provides oversight and monitors delivery of quality therapeutic services to clients/residents and discharge planning.
8. Interacts with state surveyors and resolves deficiencies by assisting in writing and implementing all Plans of Corrections cited in state surveys residential programs.
9. Assists in the agency Quality Assurance Program through monitoring and case file audits.
10. Prepares and submits Incident Reports, Reports of Abuse, Neglect and Exploitation and submits reports to appropriate authorities.
11. Maintains Survey Notebook.
12. Monitors program through frequent observation and evaluation of programs services, building, grounds, equipment, staff, clients, etc.
13. Observe residents in each home during self-administration of medications during medication administration at least once per month.
14. Responds to outside requests for persons authorized to receive information regarding the residents.
15. Provides and/or supervises crisis intervention in crisis situations as needed (assisting with a combative client, potential suicide attempt, runaway, angry family, etc.).
16. Serves on internal and external committees with approval from VPO or President.
17. Prepare and maintain department reports as assigned.
18. Coordinate with parents and guardians therapeutic home visits or absences for each assigned resident, update medical staff, house managers, Day Hab and Program Director of all assigned residents off campus arrangements with parents or guardians
19. Responsible for timely completion of assignments and adapts schedule to changing workloads to include work in evenings, weekends, and holidays when necessary.
20. Must maintain current license/certification/training in Texas
21. Participate in weekly face to face or phone supervision with the supervisor.
22. Meet all deadlines required by program supervisor
23. Attend all meetings as assigned by Vice President of Operations.
Measurable Deliverables:
1. Residents are appropriately placed and individual plans for resident care outcomes are developed and tracked.
2. Programs maintain compliance with licensing standards and compliance documentation is properly prepared and submitted promptly.
3. Develops, coordinates, and monitors Active Treatment programs for assigned residents on case load working in close consultation with Inter Disciplinary Team (IDT) in case management of residents
4. Weekly consultation with assigned Residential Direct Support Professional to ensure service and deliverables are being met.
5. Assist each resident toward the achievement of active treatment goals.
6. Supervise residents in each home during self-administration of medications, as needed and document on agency approved documentation to include active treatment as appropriate.
7. Document resident behaviors on appropriate forms.
8. Assist House Manager with conducting required fire drills and emergency preparedness drills as needed.
9. Maintain BVT professional and ethical standards of conduct outlined in FDF employee handbook including showing respect for agency staff, the service population, and community members and complying with required dress code.
10. Meet all federal and state regulatory guidelines and standards that are applicable to this position.
11. Maintain a case load in accordance with agency policies and procedures and licensing and contract standards including meeting with residents on regular bi-weekly scheduled appointments.
12. Participate in workshops, seminars, education programs and other activities that promote professional growth and development.
13. Maintain a minimum number of weekly contacts with residents and families.
14. Continually assess ongoing changes in behavior, circumstances or conditions that may affect resident safety.
15. Provide feedback and support to ensure an appropriate response to calls, families’ needs are addressed through best support, and follow-ups are conducted as needed.
16. Engage and involve residents and their family/legal guardians in the casework process.
17. Document progress that resident and their family/legal guardian toward the achievement of service goals and positive case outcomes.
18. Compile, prepare, submit and maintain accurate records, files, forms, statistics, and additional information in accordance with agency policies, licensing, and/or contract requirements.
19. Participate in Administrator on Duty Responsibilities, involving handling after-hours calls or crisis situations to guide personnel, family members, residents, etc.
20. Work evenings, weekends, and holidays to ensure coverage is provided as needed.
21. Implement BVT safety protocols including evacuating with residents and other staff in case of an emergency.
22. Maintain confidentiality in all areas of the service population and program operations.
Other Responsibilities:
1. Perform other job duties as assigned by the supervisor.
2. Attend Psychological appointments.
3. Attend other appointments if medical staff is unavailable.
Credential, Skills and Abilities:
1. Possess a valid Texas drivers’ license, work eligibility status, and have a good driving record.
2. Provide proof of work eligibility status upon request.
3. Must pass all pre-employment background checks including Criminal History, Employee Misconduct, Nurse’s Aide Registry, driving check and pass annual background checks.
4. Pass a pre-employment drug screen and random drug screen throughout employment.
5. Communicate promptly with Director of ICF-IID, House Manager and /or RN regarding needs and problems of individual ICF-IID residents.
6. Demonstrate a working knowledge of all Residential Services policies and procedures.
