Education:       Required  - Bachelor or undergraduate  degree from  an accredited university,  in 
one  of  the  following  areas:  Social  Work,  Sociology,  Psychology,  Counseling,  or Criminal 
Justice.

Experience:     Required - 2 years of experience providing direct case management services. A 
Master’s degree in a social service-related field may be used to substitute for one year of 
experience. Required- 1 year of experience providing intensive in home- based interventions.

Summary:   The Case Manger is responsible for using sound professional judgement and best 
practices to provide case management to the service population including crisis intervention as 
needed while maintaining compliance with established state and federal standards.

Essential Functions:
1.   Provides intensive home-based intervention case management services using a Trauma- Informed 
Care approach to provide service planning and documentation of services for
assigned caseload.

2.   Provide crises intervention responding promptly and professionally to crises situations with 
clients at the office, during home visits and on phone calls (within business hours) using 
independent judgement and discretion.
3.   Develop, monitor, and review Individual Treatment Plans designed to increase strengths, 
address needs, and reduce conflict in the home.
4.   Provide linkages to community resources for clients to assist with enrollment in public 
assistance, childcare assistance, employment assistance, medical needs, and support in obtaining 
critical documents such as birth certificates and SS/ID cards and transportation needs.
5.   Assist in locating appropriate housing for clients upon exit from the Transitional Housing 
program.
6.   Mandated reporter - Report any suspected abuse or neglect to the program supervisor and all 
appropriate state and county enforcement agencies.
7.   Attend and participate in; staff meetings, in-service trainings and UCAP events.
8.   Participate in weekly face to face or phone supervision with the supervisor.
9.   Frequent travel to assist clients with community resource connections as needed.
10. Meet all deadlines required by program supervisor and state/federal partners.

Measurable Deliverables:
1.   Meet all federal and state regulatory guidelines and standards that are applicable to this 
position.
2.   Maintain a caseload in accordance with agency policies and procedures and licensing and 
contract standards.
3.   Provide services to youth enrolled in the Transitional Housing Program.
4.   Complete  training  and  received  ongoing  education  on  skills  necessary  for  conflict 
resolution, anger management, parenting, etc.
5.   Conduct follow up calls and/or visits to young adults in crises situations and complete safety 
planning as needed.
6.   Compile, prepare, submit and maintain accurate case records, files, forms and additional 
information in accordance with agency policies, licensing and/or contract requirements.
7.   Work flexible hours as needed including evenings, weekends and holidays when approved by 
supervisor or to resolve crisis situations when they arise.
8.   Implement BCFS HHS safety protocols including evacuating with children and other staff in case 
of an emergency.
9.   Maintain confidentiality in all areas of the service population and program operations.
10. Maintain BCFS HHS professional and ethical standards of conduct outlined in the BCFS employee  
policy  manual  including  demonstrating  respect  for  agency  staff,  the  service population, 
and community members and complying with the required dress code, at all times.

Credentials, Skills and Abilities:

1.   Maintain licenses and  and certificates required for the including but not limited to:
□ First Aid       □ CPR               □ LMFT, LPC, LCSW, LMFT-A, or LPC-I
□ Other                                                                                         
2.   Pass  a  pre-employment  drug  screen,  random  drug  screens  and  maintain  emotional 
control, and professional composure, at all times.
3.   Provide proof or work eligibility status upon request.
4.   Pass  a  pre-employment  and  biennial  criminal  background  and  motor  vehicle  check 
(fingerprints needed if applicant has lived out of state within the past 5 years)
5.   Demonstrate the ability to:
a.   Respond  sensitively  and  competently  to  the  service  population’s  cultural  and 
socio-economic characteristics.
b.   Communicate effectively in writing and verbally.
c.    Work in a fast-paced environment.
d.   Maintain computer literacy required to meet the responsibilities of the position.
e.   Work effectively and without intensive supervision both independently and as a member of a 
multidisciplinary team.
f.    Travel as needed.
g.   Manage time effectively and efficiently.
6.   Bilingual preferred (Spanish)
