We are looking to fill a position in Bend, Oregon located in Central Oregon.
SUMMARY OF DUTIES
The primary purpose of this position is to assess the physical and cognitive needs of Oregonians applying for, or currently receiving, long-term care services. The position also determines some financial eligibility for some long-term care programs. It is responsible for developing, coordinating, and arranging a variety of services supporting older adults and people with disabilities in receiving continuity of care.
Case Managers help develop and implement case management plans, coordinate the provision of services for community-based care and nursing home care, regularly review and update long-term care service eligibility, monitor social service plans, and identify and mitigate safety risks.
MINIMUM QUALIFICATIONS
A Bachelor's degree in a Behavioral Science, Social Science, or a closely related field;
OR
A Bachelor's degree in any field and one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing);
OR
An Associate's degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing);
OR
Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
ESSENTIAL ATTRIBUTES
Experience with managing services for individuals.
Experience interpreting and implementing policy and procedures.
Experience preparing and maintaining written narrative documentation of actions taken.
Experience interviewing to obtain information to evaluate the next steps to take in a process.
*Preference will be given to candidates that are bilingual in Spanish.

Working Conditions
General office environment. You will visit clients, hospitals, nursing homes, APD offices and other providers.
May work with individuals experiencing trauma or crisis. Some individuals may have challenges managing their emotions.
May encounter situations requiring quick decision-making to ensure the safety of oneself or others.
A current driver’s license is required and an acceptable driving record.
