A Brief Overview
Accurately perform computerized indexing, scanning, quality checks, and complete analysis review of patient health records.

Education Qualifications

  • High School Diploma / GED Required


Experience Qualifications

  • 1 year Clerical and scanning experience required. Ability to work effectively and efficiently with co-workers, physicians, other departments and customers. Required


What you will do

  • Accurately scan/index hospital and clinic patient health records and loose documents.
  • Accounts for all discharged hospital records by marking off the discharge lists.
  • Consistently demonstrates the ability to quickly locate any missing paper records or electronic documents and enter into the incident reporting system (Verge) if not located within three (3) business days of discharge date.
  • Quality analysis and second QA of all scanned documents within the third party scanning system (On Base) for accuracy of all scanned images of the patient health record.
  • Exercises independent judgment in correcting errors with ability to identify and fix document errors within the hospital and/or clinic patient health record, review of any patient health record documents sent for corrections within the third party scanning system.
  • Accuracy of 95% or higher for scanning and indexing patient health records/documents.
  • Help and assist customers with Release of Information requests and questions.
  • Purge of records with the expected date rotation.
  • Prepare fetal monitor strips for going to off site storage.
  • Check timecard daily for accuracy.
