Job Summary

Summary

Join the compassionate team at Windemere Nursing & Rehabilitation Center, where RNs and LPNs deliver patient-centered, evidence-based care that enhances the quality of life for every resident. You’ll collaborate closely with CNAs, physicians, and interdisciplinary team members to ensure each resident receives individualized attention in accordance with their care plan and Windemere’s clinical standards.

Essential Functions

Assess, plan, implement, and evaluate resident care according to individualized needs and care plans.

Administer medications and treatments safely and accurately, in accordance with physician orders and facility policy.

Supervise and mentor Certified Nursing Assistants and support staff to ensure consistent care quality.

Monitor and document changes in residents’ conditions, promptly reporting concerns to the physician or nurse manager.

Participate in resident care conferences and interdisciplinary rounds to promote effective communication.

Maintain infection control, safety, and confidentiality practices per facility and regulatory standards.

Educate residents and families regarding treatment plans and health maintenance.

Uphold resident rights, privacy, and dignity in all interactions.

Education
Associate's Degree Nursing required

Licenses and Credentials
Licensed Practical Nurse [LPN - State License] - Generic - HR Only required

Experience
Long Term /Geriatric Care experience preferred but not required. New Graduate Nurses will be considered on individual basis. 0-1 year required

Knowledge, Skills and Abilities
- Must possess a current, unencumbered license to practice in Massachusetts.
- Must be able to read, write legibly, speak clearly and understand the English language.
- Must possess the ability to make independent decisions when circumstances warrant such action in a timely fashion.
- Must possess the ability to deal tactfully with staff, patients, family members, visitors, government agencies/personnel and the general public.
- Must possess leadership ability and the willingness to work harmoniously with professional and non-professional personnel and the general public.
- Must possess the ability to plan, organize, develop, implement, and interpret the program goals, objectives, policies, procedures, etc. of the QI/IC department.
- Must maintain the care and use of supplies, equipment, etc., and maintain the appearance of the department.
- Must have patience, tact, cheerful disposition and enthusiasm.
- Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
- Current BLS certification.
- Must possess the ability to make independent decisions when circumstances warrant such actions.
- Knowledgeable of nursing and medical practices and procedures, as well as Massachusetts State laws, Federal laws, regulations and guidelines that pertain to long term care.
- Leadership and supervisory ability to work harmoniously with and supervise other personnel (ex: Certified Nursing Assistants).
- Ability to effectively communicate with members of healthcare team pertinent information as related to resident status and care.
