CENTER FOR WEIGHT MANAGEMENT AND WELLNESS
JOB SUMMARY:
The Registered Nurse (RN) works in conjunction with members of the Brigham and Women’s Hospital Center for Weight Management and Wellness (CWMW). The RN works in collaboration with the inter-professional Endocrine team and department. Patients seen by this practice are adults presenting with overweight and obesity and obesity-related medical conditions including, but not limited to, Type 2 diabetes, Sleep Apnea, Hypertension and Dyslipidemia. Care provided within this setting is defined as outpatient obesity management and outpatient obesity education.
Patient care is provided onsite, may require some local travel.
NATURE AND SCOPE OF PRACTICE:
The Registered Nurse practices and facilitates the implementation of evidence-based care standards for obesity for adult patients in the outpatient Center for Weight Management and Wellness Center-Endocrinology Clinic. The nature and scope of Registered Nurse’s practice includes patient and family assessment, diagnosis, development of a plan of care, evaluation of treatment and education. The Registered Nurse works in collaboration with clinical staff (attending, fellow, house officer, physician’s assistant, clinical nurses, nurse practitioners, dietitians, pharmacists) from outpatient obesity treatment in the CWMW clinics including 221 Longwood Avenue, Faulkner Hospital, Patriot Place (Foxboro) and any other BWH service requesting consultation from the CWMW medical weight management team.
Specific responsibilities include, but are not limited to:
- Works collaboratively with team members to provide direct care, counseling, and teaching to patients with obesity and obesity-related comorbidities.
- Provides comprehensive obesity education in both one-on-one as well as group settings, which may include travel among various primary care and specialty clinic sites.
- Provides education and advice on the management or prevention of obesity to patients who either have obesity or are at risk for developing obesity.
- Provides patient instruction in the use and administration of oral and injectable medications.
- Provides obesity care education to professional staff in primary care and specialty practices, including registered nurses, physicians, registered dietitians, and health educators.
- Collaborates with multidisciplinary team members to increase awareness and promote obesity education services throughout the Brigham network or care providers.
- Collects and reports program data to the Clinical Program Manager
- Participates in program development, policy development and committee work as determined by the Co-Director of the CWMW Working as part of the greater Endocrine Department nursing team.
- Collaborates with CWMW team members in Quality activities. Works with CWMW team members to help ensure safe transitions from ED and inpatient to ambulatory care. This may involve telephone follow-up to ensure patient safety and establishment of outpatient plan.

The Registered Nurse is a professional nurse registered in Massachusetts who is responsible and accountable for planning and providing patient care for assigned patients in accordance with Nursing Standards of Care as set forth in the Nursing Department's Clinical Practice Manual. The registered nurse demonstrates initiative, knowledge, and clinical skills in caring for the patient with complex needs. The Registered Nurse demonstrates the ability to effectively manage patients by assuming full responsibility for the assessment, plan, implementation, and evaluation of patient care and is directly responsible to a designated nurse manager or supervisor.
For newly licensed nurses, a Bachelor of Science Degree in Nursing is required.

• Accountable for assessing, planning, implementing and evaluating a plan of care for a specific patient assignment.
• Sets priorities when organizing care for patients with varying acuity.
• Maintain continuity through clear and concise (verbal and written) communication.
• Demonstrates appropriate knowledge of growth and development of the adult and geriatric patient. In areas, such as the NICU and Obstetrics the principals of growth and development are applicable to the neonate.

• Documents patient care in a manner that is clear, complete, concise and in compliance with nursing documentation standards.
• Develops a comprehensive plan of care based on data from an initial assessment of patient and family, information from other members of the health care team, intra-agency referral and previous medical records.
• Develops a comprehensive educational plan for the patient and family, utilizing appropriate resources and documents according to the department’s documentation standards.
• Develops a comprehensive discharge plan utilizing appropriate resources and referrals including community resources.
• Evaluates the effectiveness of the plan of care, and documents progress in meeting stated goals. Revises plan of care as needed to achieve desired outcomes.
• Participates actively in the unit-based and organizational quality management and/or quality improvement programs.
• Provides a safe environment for patients, staff, family and visitors.
• Administers medication safely according to established policies and procedures.
• Performs nursing procedures safely and efficiently.

Assesses patient's acuity accurately when making decisions regarding staffing, • transfers and assignments. Demonstrates awareness of the need to manage within established budgetary boundaries.

• Participates in the orientation of new staff members, students and others as appropriate.
• Provides input into the clinical evaluation of other staff, as appropriate.
• Assumes responsibility for the unit in the absence of leadership personnel.
• Adheres to hospital and nursing policies and aids in their interpretation to others. Recommends change in policies and procedures through appropriate channels.
• Participates in the cooperative effort and peer support required for the smooth running of the unit, e.g., flexibility in relation to patient assignments, shift assignment, or work schedule.

1. Minimum of 3 years of clinical nursing experience in an Ambulatory Clinic/Practice setting required. Experience only in an ambulatory procedural area or emergency room not sufficient.
2. Minimum 1 year experience with telephone triage and telephone-based clinical assessment techniques is required. This is the safe, effective, and appropriate disposition of health-related problems and includes: assessing the severity of symptoms, communicating urgent patient needs to the LIP, referral to appropriate setting or level of care, documenting interventions, and an individualized plan of care using an EMR, education of patients and families on their disease process and interventions to promote health and well-being, preliminary interpretation of data, providing home health advice and follow-up, ability to make triage decisions quickly and calmly in emergency situations, and assuring continuity of care.
3. Experience with in-person triage and clinical assessment techniques of an adult patient within the last 4 years is required.
4. Weight management, endocrinology, or metabolic health experience (within last 4 years) strongly preferred.
