RESPONSIBILITIES
As an Anticoag RN, your responsibilities will include:
- Delivering direct patient care and support, ensuring high nursing standards.
- Conducting patient assessments, devising care plans, and assisting in outpatient procedures with a focus on patient comfort and safety.
- Instructing patients and families about treatment plans, medications, and care following procedures.
- Collaborating closely with healthcare professionals to coordinate comprehensive patient care.
- Upholding infection control protocols and maintaining a safe environment for patients and staff.
- Demonstrates an understanding of the coagulation process, the role of Vitamin K, thrombogenesis, hypercoagulable states and antithrombotic therapy.
- Identifies indications for anticoagulation therapy.
- Identifies EBP goals INR for patients on warfarin.
- Communicates to AMS NP when indication for or goal INR is unclear or is outside of EBP.
- Understands the mechanism of action of warfarin and other anticoagulants used in clinic setting.
- Assesses and manages anticoagulation with respect to patient medications, disease states, diet and lifestyle/activity.
- Understands and properly uses the Point-of-Care meter
- Identifies need for and performs QC on the Point of Care meter
- Understands the meaning of prothrombin time (PT), International Normalized Ratio (INR), and International Sensitivity Index (ISI) and how they relate to each other.
- Provides care to patients at AMS INR clinics and the potential to travel to different INR clinic sites. Provides care to patients remotely (requires AMS CNP approval and MGB compliant device).
- Utilizes IDTF web portals for patients who perform self INR testing from home
- Utilizes Inbasket in Epic & external labs to obtain INR results.
- Interprets INR and adjusts warfarin dosing, using practice guidelines and clinical assessment and clinical judgment for each patient.
- Collaborates with AMS CNP when care falls outside of practice protocol, complex medication interactions, critical high or low INRs
- Provides ongoing education to patients regarding warfarin, other medications, health status, diet, activity and lifestyle
- Assesses for signs and symptoms of bleeding, thromboembolism, falls.
- Records all INR’s, assessment findings, anticoagulation medication doses, next INR date in the medical record.
- Schedules INR appointments at the INR clinic(s) or lab. May adjust appointments within protocol for return INR to meet patients’ needs, i.e. rehab, MD appointments, device checks, research, work/driving issues.
- Updates Epic &/ or the AMS episode with pertinent patient care information.
- Communicates to CNP when a patient is scheduled for invasive cardiovascular or other procedure, surgery.
- Assists with transitions in care anticoagulati
on management (e.g. hospital, STR, VNA/ home health care, hospice)
- Pends anticoagulation medication orders/ refills to AMS NPs
- Communicates and coordinates with other providers regarding mutual patients to provide continuity of care.
Education
Associate's Degree Nursing required and Bachelor's Degree Nursing preferred
Licenses and Credentials
Registered Nurse [RN - State License]
BLS required
Experience
Clinical nursing experience 0-1 year required
Anticoagulation care or cardiac experience 1-2 years preferred

We are currently hiring a full-time, 40-hour Registered Nurse to work on site at our 30 Locust Street location in Northampton, Monday-Friday between 7:30 am and 4:00 pm!
