The Perinatal and Early Childhood Social Worker will provide clinical and case management care to pregnant individuals and young children in an outpatient clinical setting. Conduct individual, group, and family therapy. Provide emotional and psychological support to expectant and new parents. Provide clinical support to children 0-5 and their caregivers. Conduct assessments to identify the needs of families during the perinatal and early childhood period. Develop and implement care plans based on individual family needs. Offer crisis intervention services when necessary. Coordinate with healthcare providers to ensure comprehensive care. Educate parents on infant care, breastfeeding, and postpartum recovery. Provide bereavement support to families experiencing loss. Maintain accurate and timely documentation of services provided. Participate in case conferences and interdisciplinary team meetings. Participate in community outreach and committee meetings.

Requirements:

• Master's degree required.

• NYS Registration/Certification as a Social Worker or Mental Health Counselor (LMSW/ LCSW/LMHC) required.

• Prior experience in a hospital or community health agency is preferred.

• Experience with assigned patient population is required.

 
Salary Range: $41.694/hr - $50.878/hr

Schedule: M/T/F - 9AM - 5PM, T/TH 11AM - 7PM (subject to change with notice)
