Job Description:
We are looking for a Resident Services Coordinator to join our team! The individual in this key position will be responsible for maintaining and improving the viability of the housing community and quality of life for residents. Services should increase resident access and eligibility to community resources, build a sense of community and facilitate resident participation in programs that engage physical, social and mental wellbeing.
Location: Rochester, NH
Schedule: Part time 8 hours a week (two 4-hour shifts per week)
Pay: $21/hour
Responsibilities include:
- Perform needs assessment and document interactions and referrals in appropriate electronic platform for residents, as needed.
- Assist, educate and refer residents (and families as appropriate) to services which may be necessary to maintain a self-reliant lifestyle including but not limited to entitlement programs such as SSA, food stamps, tax assistance, medication assistance and transportation.
- Track resident referrals to community programs and services. Document interactions and referrals made for residents. Monitor to ensure resident satisfaction with service delivery and impact.
- Promote health and wellness activities for all residents. Provide coordination and support for onsite social programming and health promotion by organizing scheduled program sessions and other special events as may be desired by residents. Use focus groups, one-on-one interviews and surveys to determine resident interests, issues and service as needed.
- Encourage residents to be proactive in meeting their social, psychological and physical needs.
- Assist residents in building informal support networks among themselves and with family members/natural supports, develop a sense of fellowship and community among residents using community organization and facilitation skills. Support and encourage resident efforts in community building initiatives.
- Develop an inventory of local health and social services for use by residents, families and caregivers.
- Act as a liaison between community agencies, service providers, and housing community.
- Refer and/or oversee additional support and follow-up service for any resident experiencing problems that may threaten health, safety, and continued independent living.
- Facilitate meeting needs of residents when necessary, while avoiding potential unhealthy dependence.
- Use the least restrictive intervention necessary to alleviate a problem situation.
- Upon the request of the housing manager, act as a resource to assist residents or coordinate training for residents in understanding tenancy obligations.
- Work as a team member with housing manager in servicing residents and staff; not to include performing any duties or functions that are assigned to management or are associated with management responsibilities.
- Maintain professional boundaries with all current, past, and prospective residents, and maintain the confidentiality of residents and staff.
- Responsible for having adequate knowledge of all Easterseals programs and will gather sufficient intake information to make referrals to Easterseals programs that are beneficial to the resident.
Qualifications:
- Bachelor of Social Work or degree in Gerontology, Psychology, or Counseling is preferable.
- Minimum High School diploma or equivalent.
- Four (4) years of related experience required
- Demonstrated working knowledge of community services in the Greater Rochester area
- Proven experience in service management or facilitation, including organizing, problem-solving and advocating
- General knowledge of the aging process, older adult services, disability services, substance abuse, mental health services and human services
- Knowledge of eligibility for the application procedures for federal and state entitlement programs
- Good communication skills, writing, oral, problem solving and organizational skills in addition to strong advocacy capabilities
- Valid driver’s license and reliable transportation
