Job Summary

Coordinates the care and services of patients across the continuum of their hospitalization. The case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs, which would achieve optimal clinical and resource outcomes.
 

Qualifications

• Graduate of NLN Accredited School of Nursing Required or BSN preferred or
• Masters Degree from an accredited school of Social Work required
• Minimum of one year (1) years experience in an acute care environment
• Ability to read, write, and communicate in English.
• Strong clinical, organizational, and interpersonal skills.
• Registered Nurse - New York State Board of Nursing Current New York State Licensure as a Registered Professional Nurse required or
• New York State Licensure to practice Social Work within 6 months after hire date
• Certification in Case Management required 18 months after eligibility. NYS Screen certification required 3 months of hire
