Job Summary

The Registered Nurse, in collaboration with the Clinical Nurse Manager, is responsible for providing quality patient care by demonstrating clinical competence and participating in ongoing Performance Improvement. Provides professional nursing care utilizing the nursing process, with special attention to the special needs and behaviors of the total population. Manages the provision of patient care through integrating the functions of the Health Care Teams. Assumes responsibility for clinical leadership on the unit as assigned by the Clinical Nurse Manager.

Essential Functions
  • Recognizes the importance of patient/staff safety precautions as it relates to their specific department/area of responsibility. Consistently utilizes precautions and follows appropriate procedures to enhance patient/staff safety in their daily operations. Identifies and reports any potentially dangerous situations to their manager which could cause medical error/patient or staff harm. Immediately takes action, where appropriate, to minimize the risk of injury. Where appropriate to staff member’s responsibilities, reports errors, when these occur, to their supervisor and on an occurrence report. Participates, when appropriate, in Root Cause Analyses and/or other efforts to reduce risk of medical error/patient or staff harm.
  • Participates in performance/customer service improvement; actively assists in improving performance based on results obtained from customer satisfaction/PI activities. Recognizes systems approach to PI. Where appropriate, participates in Performance Improvement activities including team membership. Identifies processes that need improvement.
  • Performs a thorough and ongoing nursing assessment including appropriate admission assessment of patient and support system.
  • Identified and priorities current and potential problems, reassessing patient and plan.
  • Implement plan of care at admission initiating referrals as necessary considering cultural, spiritual, psychosocial and age related needs.
  • Collaborates with appropriate health team members, patient and family/significant others in developing, implementing, evaluating and revising the patients plan of care Q 24 hours or as warranted by the patients condition.
  • Prepares patient/family for discharge and follow-up.
  • Identifies and documents barriers to learning.
  • Demonstrates the ability to process information on the plan of care and provide individualized, ongoing education to the patient and family/significant other throughout the continuum of care.
  • Performs all treatments and procedures in accordance with established Policies and Procedures and Patient Care Standards
  • Accurately documents pertinent patient information, reflecting optimal patient outcomes
  • Accurately completed hospital-related information in accordance with Policies and Procedures (ie: incident reports, time request forms, ADR etc.)
  • Provide to the patient the following services: administration, monitoring, pre and post assessment evaluation of the patient’s response to the medication administration via any route.
  • Demonstrates ability to articulate issues with other staff
  • Formulates patient care assignments for various employee and identifies components of the plan of care that can be delegated to allied personnel.
  • Informally evaluates the performance of employees assigned to the unit and offers recommendation concerning their performance.
  • Serves as a resource person/preceptor for new orientees.
  • Demonstrates receptiveness towards promoting own growth and development.
  • Is proficient in demonstrating the procedure for disaster plans and protocols
  • Checks crash cart and documents.
  • Demonstrates proficiency when called upon to both life safety/emergency preparedness situations.
  • Uses proper body mechanics when moving or transporting patients and handling equipment.
  • Actively supports and adheres to all safety and infection control guidelines and procedures within the department.
  • Demonstrates knowledge and integration of OSHA Blood-borne Pathogen and adheres to all safety handling of hazardous materials.
  • Gives/receives appropriate information during change of shift report.
  • Participates in interdisciplinary rounds.
  • Enters and retrieves computer information ie: enters orders, receives orders, and obtains patient reports.
  • Carries out physician orders ie transcribing, clarifying any unclear orders and faxing orders to pharmacy.
  • Communicates to the physician the patient’s response to therapy and whether objectives are being met.
  • Gives specific consideration of patient’s age related needs during processes of assessment, treatment, and care.
  • Demonstrates knowledge of human growth and development in patient care management and adjust plan of care accordingly.
  • Demonstrates age appropriate communication skills during patient interaction.
  • Performs other related duties as assigned.

Qualifications
  • BSN Graduate of an accredited school of nursing. Required
  • Ability to read, write and communicate in English
  • Demonstrates basic knowledge of nursing theory and practice.
  • Knowledge of appropriate technology and practice standards for area of responsibility.
  • Demonstrates sound clinical skills, understanding of the acute care process, standards of practice and positive communication skills.
  • Registered Nurse New York - New York State Board of Nursing Required
  • BLS - American Heart Association Required
