What You’ll Do:

• Direct physical therapy treatment and evaluate home environment.
• Assess and evaluate therapeutic, rehabilitative, and functional status for the development of the total plan of care. 
• Provide patient, caregiver, and family education and counseling.

As an PT at Connected, you’ll be a maverick of creativity, a crusader for compassion, and a trailblazer in personalized care! Here’s what we're looking for:

• Active Physical Therapist (PT) licensure in Oregon
• 2+ years of Physical Therapy experience
• Critical thinking skills and ability to work independently.
• Hold a current BLS certification from the American Heart Association
• Zoom around town in your trusty insured automobile
