Senior Nurse Practitioner Geriatric Primary Care (Mobile/Field-Based)

Location: Greater Las Vegas, Nevada Area (Field-Based)

Reports To: Nurse Practitioner Manager

We are seeking a highly experienced and dedicated Senior Nurse Practitioner to serve as a clinical mentor and subject matter expert within our mobile geriatric primary care team. This is a dynamic field-based role (75% clinical, 25% mentorship) focused on providing comprehensive, integrated care to a vulnerable patient population in residential settings across the Greater Las Vegas area.

This position is purely a clinical leadership and mentorship role. The Senior Nurse Practitioner will not be involved in Human Resources (HR) management, disciplinary action, or administrative human resource issues.

Key Responsibilities:

1. Advanced Clinical Practice (Approx. 75% Time Allocation)

• Manage and round on a census of patients residing in Assisted Living, Memory Care Units and Group Homes.

• Provide advanced primary care, including assessment, diagnosis, prescribing, and management of complex co-morbidities.

• Demonstrate expert knowledge of chronic disease management and the principles of managed care in a mobile setting.

• Ensure the highest quality of patient outcomes through evidence-based practice and continuity of care.

2. Clinical Mentorship & Development (Approx. 25% Time Allocation)

• Act as a Subject Matter Expert (SME), providing clinical guidance and support to a team comprised entirely of fellow Nurse Practitioners.

• Mentorship and training of new and existing team members to ensure consistency and excellence in clinical practice.

• Actively participate in the onboarding and selection of new clinical staff.

Qualifications & Requirements:

• Active, unencumbered Nurse Practitioner license and board certification.

• Minimum of 3 years of experience managing complex geriatric and chronic medical conditions.

• Experience in geriatric, memory care, or assisted living settings is highly desirable.

• Demonstrated ability to function autonomously and effectively within a mobile/field-based practice.

• Exceptional teaching, mentorship, and communication skills.

• Must be comfortable with daily travel throughout the Greater Las Vegas area.

• Satisfactory completion of a Tuberculosis test is a requirement for this position

Compensation: $126,192K - $189,288K & bonus eligible
