Hours: Monday - Friday 8a-5p
Are you a compassionate behavioral health professional passionate about supporting individuals in recovery? Join our team at Centerstone as a Care Coordinator!
In this role, you’ll provide comprehensive service coordination and individualized support for clients participating in MAT programs. You will help clients navigate recovery challenges by connecting them to mental health, medical, and community resources while fostering stability, empowerment, and long-term wellness.

🧭 Client Care & Coordination
Review assessments and discharge planning sessions to identify needs related to employment, education, healthcare, social connectedness, and criminal justice.
Coordinate outpatient mental health and/or substance use disorder services as needed.
Make or assist with outside referrals for services not addressed through treatment.
Assist with scheduling treatment appointments and transportation arrangements.
Support the continuous development and implementation of individualized treatment plans.
🤝 Collaboration & Communication
Maintain regular communication with referral sources, primary care providers, and internal teams regarding client progress and transition planning.
Request and review documentation from previous treatment and primary care providers.
Participate in team staffing and interdisciplinary meetings to ensure continuity of care.
Represent the agency professionally in all interactions.
🏠 Family & Community Engagement
Link families to community-based services and advocate on their behalf.
Assist clients and families with transitional planning to promote successful recovery outcomes.
📋 Documentation & Compliance
Complete and maintain all required documentation in accordance with KDHE/JJA standards.
Correct chart deficiencies within required timeframes.
Provide crisis intervention and follow-up as necessary.
Adhere to weekly direct service expectations.
🎓 Professional Growth & Accountability
Maintain at least 32 hours of professional development training every two years.
Participate in supervision, team meetings, and quality improvement initiatives.
Demonstrate commitment to the organization’s mission and goals.
Perform other duties as assigned by the Clinical Supervisor, Program Director, or Executive Leadership.
Knowledge of case management and rehabilitation methods in behavioral health and substance use settings.
Understanding of the challenges faced by individuals with mental illness and/or substance use disorders.
Familiarity with legal, ethical, and procedural standards in behavioral healthcare.
Ability to evaluate and provide feedback on service quality and client outcomes.
Strong written and verbal communication skills.
Organizational and documentation skills to ensure timely and accurate recordkeeping.
Bachelor’s degree from an accredited college or university in a human services field (e.g., social work, psychology, counseling, criminal justice, human development, nursing, or education) preferred.

Must provide evidence of being free of infectious and contagious diseases prior to hire and annually thereafter.
TB Testing: Required for all direct care staff; mandatory for Kansas Residential staff.
Annual Health Assessment: Mandatory for Kansas Youth Residential staff.
Successful completion of background, driving, abuse/neglect, and fingerprint checks.
Completion of New Hire Orientation and all required Relias training upon hire and annually thereafter.
Current driver’s license, acceptable driving record, and auto insurance.
Obtain and maintain:
First Aid Certification (within 3 months of hire and at renewal).
CPR Certification (within 3 months of hire and at renewal).
Behavioral Management Training: CPI for Residential programs or Peaceful Intervention/Verbal De-escalation for Outpatient programs within 30 days of employment.
Completion of “Direct Service Expectations” within 30 days of employment.
Maintain extensive knowledge of agency policies, procedures, and applicable regulations.
Must meet driver’s license requirements in the state of residence for client transportation.
ADA Consideration – Sedentary Work
Exerting up to 10 pounds of force occasionally and/or negligible force frequently to move objects.
Frequent repetitive movements of hands, fingers, and arms for typing and documentation.
Work involves primarily sitting but may include occasional walking or standing.
