In this position, you’ll work directly with individuals in the community, helping them navigate daily challenges and build the skills necessary for long-term stability. You will develop personalized treatment plans, implement behavioral strategies, and coordinate care with a variety of professionals. Key Responsibilities:
· Support individuals in their homes and communities, helping them build skills and achieve their goals
· Collaborate with clients, families, and care teams to create individualized, strengths-based treatment plans
· Deliver evidence-based interventions including Motivational Interviewing, Trauma-Informed Care, CBT, and DBT techniques
· Facilitate or co-facilitate PSR group sessions aligned with client goals
· Coordinate care across multiple systems including primary care, psychiatry, schools, and community agencies
· Maintain detailed, timely documentation of client interactions and progress
· Engage in professional development and stay current on mental health best practices
· Participate in quality improvement initiatives through data collection and reporting
· Build trusting relationships with clients, families, and professionals to support effective care delivery
· Perform other duties as assigned by supervisor

Bachelor's degree in one of the following fields of study: Psychology, Social Work, Sociology, Education, Criminal Justice, Family Studies, Counseling, Recreational Therapy, Human Services, Human Development, Child Development, Gerontology, Behavioral Science, Rehabilitation Counseling

Associate of Applied Science in Behavioral Health Support degree from an approved institution

Work experience in a related human services field may substitute for educational requirements. *Related fields may include direct support such as case management, residential support, developmentally disabled support, vocational rehabilitation, and more. Four years of qualifying experience minimum or a combination of education and experience equaling four years required.

Qualified Addictions Professional (QAP) - CADC, CRADC, CRAADC, CCJP, RADC-P, RADC, CCDP, CCDP-D

Additional Qualifications:
· Must be at least 21 years of age to operate a company-owned vehicle
· Must be at least 23 years of age to operate a company-owned 15-passenger van
· Must have a valid driver’s license, an acceptable driving record, and current auto insurance
· Must have reliable transportation to transport clients in a personal vehicle
· Adequate written skills to accurately complete documentation within required timeframes
· Skilled in case management and community-based support
· Ability to problem-solve and communicate effectively both verbally and in writing
· Ability to multitask and coordinate services from a variety of community resources
· Ability to follow direction, accept supervision, and work independently
· Experience working with individuals experiencing mental health, substance use, or developmental disabilities is preferred

Physical Requirements:
· Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently
· Repetitive hand, finger, and arm movements for typing and writing
· Sitting most of the time with occasional walking or standing
