Position Highlights

Provides clients with continuing health care needs with a high standard of individualized health care in their homes.  Provides care in a manner to restore the client to full health or to achieve maximum rehabilitation with the least possible disruption to his/her usual pattern of daily living.  Demonstrates the knowledge and skills necessary to provide care for the population served in the department.
