Job Summary:

Independently performs professional work in a public housing assistance and family self-sufficiency program through a case management approach by coordinating, implementing, and monitoring individualized barrier removal and career development plans; and performs other related duties as required.

Examples of Duties:

  • Conducts interviews and collects data regarding personal history, social situations, and health barriers to employment, preliminary employability information related to prior work history, skills training that has been received, and basic employment interests;
  • Conducts needs assessment and develops action plans to assist program participants;
  • Implements the Family Self-Sufficiency Program plan and follows-up with service providers and other Housing Agency staff members, assuring services are adequately and appropriately provided;
  • Links clients and family members to needed services;
  • Monitors and evaluates clients' participation and progress;
  • Works directly with the client on a continuous basis to sustain a positive relationship;
  • Manages assigned cases and maintains case records and other documents in accordance with established procedures;
  • Provides instruction in the areas of life skills and pre -employment preparation to clients;
  • Interprets agencies policies and procedures;
  • Maintains cooperative working relationships with referral resources, employers and service providers including individuals, public and private agencies, through personal visits and telephone contact; and
  • Performs other related duties as required.

Minimum Qualifications:

Preferred Training and Experience: 

A combination of education and experience substantially equivalent to graduation from an accredited college or university with a baccalaureate degree in psychology, sociology, social welfare, social work, or a related field; and

  • two (2) years of specialized work experience.

Qualifying Specialized Work Experience:

Professional work experience in the field of social work, individual or family counseling which involved assisting individuals and/or families resolve personal, social, employment, and/or emotional problems.
