We are looking for an experienced, talented, and compassionate Director of Operational Excellence for a full-time position. 

The Director of Operational Excellence provides operational leadership for the agency’s performance improvement, quality and process optimization initiatives across home care and hospice services. This position drives the culture of continuous improvement through data-driven decision-making, LEAN methodology, and change management principles to enhance the quality of care, operational efficiency and organizational performance.

Salary: 135,000.00 - 150,000.00 commensurate with experience

What will you do:

Leadership and Program Oversight

• Develop, coordinate, and implement the agency’s Quality and Performance Improvement Program for home care, Palliative and Hospice services.
• Oversees coding, OASIS and HOPE completion and quality outcomes associated.
• Direct and facilitate Operational Excellence initiatives in support of organizational strategy, performance goals, and continuous improvement.
• Participate in annual goal-setting for the agency.
• Provide consultation on process redesign, workflow optimization, and readiness assessments for major initiatives and regulatory compliance efforts.
• Partner with the Chief Operating Officer on high-priority projects and operational initiatives.
• Continuous Improvement and Process Optimization
• Promote consistent, organization-wide adoption of Operational Excellence methodologies, standards, and tools that align with agency needs.
• Lead Performance Improvement Projects (PIPs) using LEAN methodology and data-driven insights to identify, implement, and sustain process improvements.
• Utilize balanced scorecard metrics and problem sheets to track and measure progress on performance improvement goals.
• Oversee the Quality Dashboard to ensure accurate monitoring, reporting, and communication of key performance indicators.
• Conduct program evaluations and operational assessments to ensure sustainable improvements and efficiency gains.

Data Analytics and Quality Management

• Review and analyze quality, compliance, and performance data (e.g., SHP reports, OASIS metrics) to identify trends and implement corrective action plans.
• Oversee audits of clinical records and data collection activities to ensure accuracy, compliance, and improvement readiness.
• Manage and interpret data to inform decision-making and guide improvement initiatives that enhance patient care delivery and operational outcomes.
• Mentor, coach, and develop managers and administrative staff, fostering professional growth and leadership capability.
• Build empowered, cross-functional teams that promote collaboration, accountability, and open communication.
• Collaborate with clinical directors, education, and other departments to standardize processes, share best practices, and align goals across service lines.
• Partner with Clinical Education staff to ensure compliance with documentation standards and regulatory requirements.
• Model and promote effective interdepartmental communication and cooperation throughout the organization.

Compliance and Accreditation

• Serve as Project Manager for certification surveys, engaging staff at all levels in preparation, education, and execution.
• Oversee quality and compliance trends, ensuring timely resolution of issues and implementation of preventive measures.
• Draft, recommend, and implement administrative policies, procedures, and processes to support the mission of the Operational Excellence Department.
• Represent the agency with external organizations such as the Home Care Alliance, attending meetings, webinars, and educational sessions to maintain current knowledge and best practice.
• Performs other duties as assigned.

What we are looking for in you:

Qualifications

• Bachelor’s degree in Healthcare Administration, Nursing, Business, or related field required.
• Minimum of 7–10 years of progressive leadership experience in healthcare operations, performance improvement, or quality management.
• OASIS and coding certification required
• Strong knowledge of LEAN, Six Sigma, and other process improvement methodologies; certification preferred.
• Demonstrated success in strategic planning, operational assessment, and data-driven decision-making.
• Proven ability to lead change, build teams, and collaborate across diverse departments.
• Exceptional analytical, communication, and organizational skills.
