Eliot’s Homeless Services Division is seeking a dedicated Team Leader to join our Intensive Case Management Community Support Program for Homeless Individuals which provides housing stabilization support to unsheltered adults who are in transition to housing pre- and post- move in.  Eliot is committed to a Housing First Model that prioritizes permanent housing as the initial step in assisting individuals experiencing homelessness.

In this role you will provide person-centered, strengths-based case-management. The ultimate goal of our programming is to provide each individual with coaching, education, tools and access to community resources that will help them successfully maintain long-term sustained housing, financial independence and emotional well-being.

Responsibilities:

  • Support the Director of Intensive Case Management Services with clinical and administrative oversight including program growth/expansion.
  • Provide onboarding support and training to new hires related to policies, procedures and best practices.
  • Provide real-time guidance, consultation and consistent weekly supervision to assigned Housing First Coordinators and Recovery Coaches – including side-by-side experiential learning in the field.
  • Provide comprehensive assessment and triage of incoming Intensive Case referrals.
  • Liaise with internal/external partners regarding service needs and risk.

Qualifications:

  • Requires a Bachelor’s degree in social work, counseling, psychology, nursing, or another closely related field.
  • At least two years direct experience with underserved populations and at least one year of managerial experience.
  • Demonstrated ability to identify and access community resources.
  • Must have a valid driver's license and reliable transportation.
  • Master’s degree in healing arts preferred.

Schedule: Monday-Friday, 9am-5pm

 

Annual Salary
$75,000$85,000 USD
