$24.22 - $33.91The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
The social work assistant is responsible for the implementation of psychosocial assistance and for ensuring continuity of care for Strong Ties patients. They participate in the planning and delivery of appropriate service linkages, and serve as a liaison and advocate for patients and their families with identified service providers. The Social Work Assistant is also a member of the interdisciplinary treatment team for patients at Strong Ties Clinic.ESSENTIAL FUNCTIONS
