$24.22 - $33.91The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
The social work assistant coordinates functions associated with patient information processing for the Child and Adolescent Partial Hospitalization Service (CAPHS). Assists and acts as a resource to clinical staff providing care to patients, ensure continuity of care, and support compliance with treatment. Monitors and ensures that all tasks assigned are completed in an accurate, efficient, and customer friendly manner. Performs functions associated with gathering and processing patient information for ambulatory care visits. Understand and have an awareness of potential presenting problems, mental diagnosis, and the unique needs of the patients and their families. This includes, but is not limited to the need for de-escalation, sensitivity, triage, and referral. Completes the responsibilities of reception, registration, e-Record task management, and program referral management via EMR referral work as well as other methods of incoming referrals.ESSENTIAL FUNCTIONS
